Application for Wood Processing Plant Permit

 

Requirements 

  1. Duly accomplished Application Form
  2. Application Fee
  3. Certified Photocopy of the Certificate of Registration from appropriate Government Agencies
  4. For Individuals, Proof of Filipino citizenship
  5. Environmental Compliance Certificate (ECC) or Certificate of Non-Coverage (CNC)
  6. Sworn Statement of the applicant, declaring the source of wood of raw materials
  7. Endorsement from the Local Government Unit
  8. In case the Wood Processing Plant is/will be located in a public forest land, SLUP and WPPP will be processed simultaneously

Application for Tree Cutting Permit

 

Requirements

  1. Duly accomplished Application Form
  2. Authenticated Copy of Ownership to the Land
  3. Inventory fee based on DAO 2004-16
  4. Harvesting Development Plan
  5. Endorsement from any of the following Local Government Unit Officials
    1. Barangay Captain
    2. Municipal/City Mayor
    3. Provincial Governor
  6. Initial Environmental Examination (IEE) as a basis for the Environmental Compliance Certificate (ECC)

Application for Special Tree Cutting Permit

 

Requirements

  1. Filled-out Online Application Form or Letter of Application
  2. Authenticated copy of ownership to the Land (for private land)
  3. Inventory fee based on DAO 2004-16
  4. Harvesting Development Plan, if the application covers = (10) hectares with at least 50% of the area covered with forest trees
  5. Endorsement from any of the following Local Government Unit Officials
  6. Initial Environmental Examination (IEE) as a basis for the Environmental Compliance Certificate (ECC)

Application for Special Private Land Timber Permit

 

Requirements

Initial

  1. Filled-out Online Application Form or Letter of Application
  2. Authenticated copy of ownership to the Land
  3. Endorsement by Local Agrarian Reform Officer, for areas covered by Certificate of Land Ownership Award (CLOA)
  4. Inventory fee based on DAO 2004-16

Supplemental

  1. Harvesting Development Plan, if the application covers = (10) hectares with at least 50% of the area covered with forest trees
  2. Endorsement from any of the following Local Government Unit Officials
  3. Initial Environmental Examination (IEE) as a basis for the Environmental Compliance Certificate (ECC)

Application for Special Land Use Permit

 

Requirements

  1. Duly accomplished Application Form
  2. Application Fee
  3. Map of the applied area; including the technical description, longitude and latitude coordinates, and tie point from the nearest landmark
  4. Pertinent documents showing proof that the applicant is a legitimate entity qualified to be a holder of a forestland Tenurial Instrument or Agreement
  5. For an individual applicant, certified copy of Birth Certificate or, certified copy of Certificate of Naturalization
  6. For an association, corporation, cooperative or partnership; certified copy of Securities & Exchange Commission (SEC) Registration Certificate, Articles of Incorporation/Partnership, and Resolution of the corporate governing body (Board of Directors, Board of Trustees, etc.) designating the authorized representative of said corporation, association or partnership to apply/sign documents for and in behalf of the company
  7. For cooperative, certified copy of Certificate of Registration with the Cooperative Development Authority (CDA)
  8. Environmental Compliance Certificate (ECC), issued by the Environmental Management Bureau of DENR
  9. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)
  10. Clearance from Palawan Council for Sustainable Development (PCSD), if the applied area is located in the province of Palawan
  11. Proof of Financial Capability to develop and manage the applied area
  12. Endorsement from the concerned CENRO, PENRO and RED of DENR
  13. Performance bond twice the annual rental of users fee as the case maybe, but not less than P10,000. Provided that 50% of the computed bond deposit may be posted in CASH and a balance in the form of surety with a duration of (3) three years
  14. Annual rental shall be paid before the issuance of the permit and the performance bond shall be posted simultaneously
