  2. Application Fee
  3. Sketch map of the applied area
  4. If applicant is a corporation, association or partnership a true copy of its Articles of Incorporation/ Partnership and by-laws duly certified by the Securities and Exchange Commission (SEC)
  5. If applicant is a cooperative, a true copy of the Articles of Incorporation and by-laws duly certified by the Cooperative Development Authority (CDA)
  6. If applicant uses a trade name; a certificate of registration from the Department of Trade & Industry (DTI)
  7. Certified Copy of Latest Income Tax Return
  8. Proof of Capitalization
  9. Endorsement from the concerned Local Government Unit (LGU)
